Entry Forms

You can apply for the School Toilet Award online or by post.

1. There are two entry forms to fill in:
• The School Self Assessment Entry Form
• The Pupil Assessment Entry Form
2. You also need to provide supporting evidence:
• Photographic evidence of the pupils' toilets is required, preferably
  of each set of pupils' toilets.
• Other supporting evidence may be sent e.g. School Toilet Policy; Toilet Action Plan.

1. There are 5 steps to applying online:

Step 1

Enter a main contact email address - this will be used in identifying your application. You will need to enter this email address when filling in each online form.

Step 2

Fill in the School Self Assessment Entry Form.

Step 3

Fill in the Pupil Assessment Entry Form.

Step 4

Upload your supporting evidence:

• Photographs of the toilets (you can upload a maximum of 12 photos).
• Written evidence - such as your school toilet policy and action plan, if appropriate (you can upload a maximum of 3 pieces of written evidence).

Step 5

Final confirmation. Review your entry forms and submit your application.

• If you need to make any changes after you have submitted your entry, please email the necessary information using our online enquiry form.
• Please do not make another online entry form application.

The Entry Forms can be printed and submitted by post, together with your photographs and supporting evidence.

1. Please type entries or use a dark pen and ensure it is easily legible.
2. Please keep a copy of both entry forms for your files.
3. Photographs must be clearly labelled with school's name and identification e.g. Bedford High School; girls'; years 7 - 9; adjacent to classrooms.
4. Please label any supporting written evidence with school's name.
5. We regret but we are unable to return any photos or supporting evidence.
